The Embassy's consular outreach team will provide the following services;
1. Passport Renewal and First Time applicants (strictly by appointment)
2. Notarials for Affidavits and Special Power of Attorney
3. NBI Clearance Fingerprinting
4. Report of Birth, Marriage, and Death
5. Dual Citizenship

Those who wish to get a passport renewal appointment schedule are requested to contact the Philippine Embassy at 613-2331121 starting 09:00 AM of 10 April 2018 (Tuesday). As passport renewal/first time applicant slots are limited, appointment slots will be given out on a first come, first-serve basis.

Filipinos who will avail of other consular services aside from passport services need not secure an appointment slot.

BEFORE YOU GO

Applicants are requested to prepare the requirements listed below to ensure completeness of documentary requirements for the respective service going to be availed of to avoid inconvenience:

1. Passport
a) Completed e-passport application form (downloadable from http://ottawape.dfa.gov.ph/index.php/2016-04-12-08-36-34/downloadable-forms  
b) Original passport – Applicants are kindly advised to fill out the application form and photocopy the required supporting documents in advance in order to avoid disruption in the appointment schedule.
c) Photocopy of biographical data page of passport
d) Photocopy of one of the following for confirmation of Philippine citizenship: work permit/permanent resident card/current visa/certificate of citizenship acquisition (original must be presented)
e) Cash payment of C$81
f) Xpresspost envelope, one per applicant (Regional for residents of ON and QC residents and National for residents from other provinces) with signature sticker (Please write your complete return address and your telephone number/s)

ADDITIONAL REQUIREMENT: for applicants renewing BROWN/GREEN passports
- NSO Birth certificate
- NSO Marriage certificate (for women who use their husband’s surname)
For minor applicants:
- NSO Birth certificate
- Photocopy of passport data page of accompanying parents

2. Notarial Services
a) Download and present the completed document to be notarized (prescribed forms may be
downloaded from http://ottawape.dfa.gov.ph/index.php/2016-04-12-08-36-34/downloadable-forms 
b) Photocopy of data page of passport
c) Photocopy of gov’t issued ID or permanent resident card (original must be presented)
d) Photo copy of relevant papers to be attached to the notarized document
e) Cash payment of C$33.75
f) No need to book an appointment

3. NBI Clearance Fingerprinting
a) Two recently-taken passport-sized colored photographs
b) Cash payment of C$33.75
c) No need to book an appointment

4. Report of Birth
a) . Completed Report of Birth form (four originals)
b) Four copies of birth certificate (original must be presented)
c) Four copies of birth certificate of parents
d) Four copies of valid passports of parents (original must be presented)
e) Four copies of marriage certificate of parents (original must be presented)
f) Affidavit of Late Registration, if ROB is reported after 1 month from date of birth
g) Cash payment of C$33.75
If parents are not married:
- Affidavit of Admission of Paternity to be executed by the father with photocopy of father’s passport data page. Cash payment of C$33.75
- Affidavit to Use the Surname of the Father to be executed by the mother with photocopy of mother’s passport data page. Cash payment of C$33.75.

5. Report of Marriage

a) Completed Report of Marriage form (four originals)
b) Four copies of marriage license or registration of marriage (long form) issued by the Office of the
Registrar General in Canada (original must be presented)
c) If there was previous marriage by either party which was annulled, four copies of the Certificate of Finality of the Nullity of Marriage or Certificate of Finality of Judgment in the Recognition of a Foreign Decree of Divorce, both issued by a Philippine court (which should be authenticated by the Department of Foreign Affairs in Manila),
d) Four copies of spouses’ passports and birth certificates
e) Consular fee of C$33.75

6. Dual Citizenship

a) Completed Citizenship Retention and Reacquisition Application Form (can be downloaded from http://ottawape.dfa.gov.ph/index.php/2016-04-12-08-36-34/downloadable-forms )
b) Original and photocopy of Philippine Birth Certificate issued by the Philippine Statistics Authority (PSA).
c) Three recent 2"X2" colored photographs of the applicant
d) Canadian naturalization document card
e) Canadian passport
f) Fee of C$67.50
g) Xpresspost envelope, one per applicant (Regional for residents of ON and QC and National for residents from other provinces) with signature sticker (Please write your complete return address and your telephone number/s)
