REQUIREMENTS
- Applicant must be 18 years old and above.
For the issuance of a certification, please present the original documents and submit two (2) copies of the following:
- Duly accomplished Affidavit of Renunciation of Philippine Citizenship. Please click here Affidavit of Renunciation of Philippine Citizenship;
- Philippine passport – original will be canceled and will be returned to the applicant;
- For those who have retained/re-acquired Philippine citizenship (dual citizens) under R.A. 9225, submit the original Oath of Allegiance, Identification Certificate and Order of Approval – originals will be canceled and retained at the Philippine Embassy;
- Birth Certificate issued by the Philippine Statistics Authority;
- Canadian citizenship certificate / valid Canada passport;
- For those born in Canada, submit proof of Philippine citizenship of parent/s at the time of birth (e.g. Canadian citizenship certificate, permanent resident, Philippine passport).

Submission of the above documents are to be made in person at the Embassy of the Philippines, 30 Murray St., Ottawa, ON K1N 5M4, or at any consular outreach mission. Please secure an appointment at https://appointment.ottawape.org/.

WHERE TO CLAIM YOUR CERTIFICATION
If the individual opted to pay the expedited processing fee, he/she can pick them up at the Embassy of the Philippines between 3:00 p.m. and 5:00 p.m.
Non-expedited documents not for mailing may be picked up in 3 to 5 days. Clients must leave their contact details to be used in coordinating the release of the document.
If the individual opted to have the certification mailed to them, he/she will get the document via mail at his/her given address. Clients who choose to receive their documents via mail must submit a prepaid self-addressed courier envelope with tracking features (e.g. FedEx/DHL/Canada Post Xpresspost with signature sticker).

- Ontario and Quebec residents need to provide a Regional Xpresspost envelope.
- Residents of other Canadian provinces need to provide a National Xpresspost envelope.
- Residents of Bermuda need to provide an envelope from an international courier of their choice with an airway bill number.
PROCESSING PERIOD
- Expedited documents: same day processing
- Non expedited documents: 5 days (exclusive of mailing time)

- The Consular Officer reserves the right to require additional proof or documents from an applicant to prove his/her citizenship, status or identity pursuant to the Philippine Passport Law (RA 8239) and the Foreign Service Act (RA 7157).
