Consular Mortuary Certificate is required for the shipment of human remains or for carrying or shipping cremated remains. If the deceased was a citizen of the Philippines at the time of death, a Report of Death should also be filed.

WHO MAY FILE?

  1. Next of Kin of the deceased;
  2. Funeral home director

WHAT TO FILE?

ALL DOCUMENTS SUBMITTED SHOULD BE IN ENGLISH OR TRANSLATED TO ENGLISH.

FOR SHIPMENT OF HUMAN REMAINS:

*Each in one original and 2 photocopies:

1. Death Certificate issued by the Vital Statistics Office;

2. Embalmer’s Certificate stating that the body was duly embalmed in accordance with local laws, and is in sanitary condition for shipment abroad;

3. Certification from the attending physician stating 1) the cause of death; and 2) the cause of death is NOT a contagious disease

4. Mortuary Certificate stating that the casket contains only the body of the deceased and that the remains are placed in a hermetically sealed casket.

5. Burial or transit permit issued by the local provincial office;

6. Photocopy of the identification page of the deceased’s passport;

7. Flight itinerary of the remains, including the name, address and telephone number of the receiving funeral home or consignee in the Philippines, and name, address and phone number of the next of kin in Canada and in the Philippines; the name, valid passport and flight itinerary of the person accompanying the remains, if any; and

FOR CARRYING / SHIPMENT OF CREMATED HUMAN REMAINS: 

*Each in one original and 2 photocopies:

1. Death Certificate by the Vital Statistics Office;

2. Medical Certification of Death issued by an attending physician indicating the cause of death;

3. Cremation Certificate stating that 1) the remains were cremated in accordance with local laws; 2) the ashes are placed in a hermetically sealed urn or a similar container and in a sanitary condition for overseas shipment in accordance with existing standards; and 3) the urn contains only the ashes of the deceased. 

4. Photocopy of the identification page of the decedent’s passport;

5. Name, copy of valid passport and flight itinerary of the person accompanying the cremated remains to the Philippines; and

PAYMENT OF FEES:

The fee for the consular mortuary certificate is C$ 36.25 in cash or through debit card if to be paid in person, or in a postal money order / bank draft of the same amount payable to EMBASSY OF THE REPUBLIC OF THE PHILIPPINES if to be paid by mail. Applicants from Bermuda may send a bank draft in the amount of US$ 25.00

 

WHERE AND HOW TO SUBMIT YOUR DOCUMENTS

In-Person Submission

Documents for the shipment of human remains may be submitted in person at the Embassy by appointment from Mondays through Fridays, except during Philippine and Canadian holidays. Please secure your appointment at https://appointment.ottawape.org/ The Embassy is located at 30 Murray St., Ottawa ON   K1N 5M4. If the document will be picked up in person, please leave contact information to be used in coordinating the release of the document.

A prepaid self-addressed courier envelope (preferably with tracking features) must be enclosed to ensure the safe return of the applicant's documents (e.g.FedEx/DHL/Canada Post Xpresspost with signature sticker, etc.)

For Ontario and Quebec residents, please provide a Regional Xpresspost envelope; If a resident of other Canadian provinces/territories please provide a National Xpresspost envelope; and for clients in Bermuda, the applicant may submit an envelope from an international courier of their choice with the airway bill number. 

Please note the requirement of a bank draft or postal money order in the exact amount payable to EMBASSY OF THE REPUBLIC OF THE PHILIPPINES in mailing your documents.

The Embassy is not liable for any delays or loss of documents in the mail, which is a service provided by third parties.

 

PROCESSING PERIOD:

The regular processing time is within five (5) working days from the date the complete set of documents is received.
