Applicants are requested to take note of the following:
1) Application for passport renewal
To ensure an orderly delivery of service, applicants are requested to contact the Consular Section of the Philippine Embassy in Ottawa (Tel: 1-613 2331121) immediately for an appointment. Appointment slots will be given out on a first come, first serve basis. Pease provide your contact number and email address when making an appointment.
Accepted applicants are requested to:
• Come in decent attire with collar and sleeves for the photograph taking (avoid wearing shirt/blouse in light blue color). They will also be asked to remove earrings, eyeglasses, hair accessories and any facial jewelry.
• Bring the following:
a) Completed e-passport application form (downloadable from www.philembassy.ca)
b) Original passport – Applicants are kindly advised to fill out the application form and photocopy the required supporting documents in advance in order to avoid disruption in the appointment schedule.
c) Photocopy of biographical data page of passport
d) Photocopy of one of the following for confirmation of Philippine citizenship: work permit/permanent resident card/current visa/certificate of citizenship acquisition (original must be presented)
e) Cash payment of C$81
f) Xpresspost envelope (National) with signature sticker (Please write your complete return address and your telephone number/s)

2) Application for notarial services
a) Download and present the completed document to be notarized (prescribed forms may be
downloaded from www.philembassy.ca)
b) Photocopy of data page of passport
c) Photocopy of gov’t issued ID or permanent resident card (original must be presented)
d) Photo copy of relevant papers to be attached to the notarized document
e) Cash payment of C$33.75
f) No need to book an appointment
3) Application for dual citizenship
a) Completed application form (downloadable from www.philembassy.ca)
b) Photocopy of PSA (formerly NSO) Philippine birth certificate (original must be presented)
c) Cash payment of C$67.50

f) No need to book an appointment
4) NBI clearance fingerprinting
a) Two recently-taken passport-sized colored photographs
b) Cash payment of C$33.75

5) Report of Birth (ROB)
a) Completed Report of Birth form (in quadruplicate)
b) Four copies of birth certificate (original must be presented)
c) Four copies of birth certificate of parents
d) Four copies of valid passports of parents (original must be presented)
e) Four copies of marriage certificate of parents (original must be presented)
f) Four passport-sized photos of applicant
g) Affidavit of Late Registration, if ROB is reported after 1 year from date of birth
h) Cash payment of C$33.75
6) Report of Marriage (ROM)
a) Completed Report of Marriage form (in quadruplicate)
b) Four copies of marriage license or registration of marriage (long form) issued by the Office of the
Registrar General in Canada (original must be presented)
c) Four copies of the Certificate of Finality of the Nullity of Marriage or Certificate of Finality of Judgment in the Recognition of a Foreign Decree of Divorce, both issued by a Philippine court (which should be authenticated by the Department of Foreign Affairs in Manila), if there was previous marriage by either party which was annulled.
d) Four passport-sized photos of husband (groom) and wife (bride)
e) Four copies of spouses’ passports and birth certificates
f) Consular fee of C$33.75
