Additional Requirements for a Damaged Passport

  • Affidavit of Damage of Passport stating the detailed circumstances surrounding the damage to the passport, notarized by a Notary Public and authenticated by Ministry of Foreign Affairs;
  • Original damaged passport and its photocopy;
  • Photocopy of passport's data page prior to its damage, if available.
  • If photocopy is not available, and passport's data page is damaged, with the information of applicant's name, birthplace, birthdate or photo illegible, applicant must submit the applicable document:

 

  • If born in the Philippines, birth certificate in security paper issued by the Philippines National Statistics Office (NSO) and authenticated by the Philippines Department of Foreign Affairs (DFA).
  • If born outside of the Philippines, original or certified true copy of the applicant's birth certificate and its English translation that are authenticated by the Ministry of Foreign Affairs of the country where document was issued.
  • For female applicants who married outside the Philippines and if data entry on name is damaged, original or certified true copy of the marriage certificate and its English translation that are authenticated by the Ministry of Foreign Affairs of the country where the marriage was solemnized.
  • For female applicants who married in the Philippines and if data entry on name is damaged, original or certified true copy of marriage certificate issued by the Philippines National Statistics Office and authenticated by the Philippines Department of Foreign Affairs.
  • For widowed female applicants whose husband died in the Philippines, death certificate in security paper issued by the Philippines National Statistics Office (NSO) and authenticated by the Philippines Department of Foreign Affairs (DFA).
  • For widowed female applicants whose husband died outside the Philippines, original or certified true copy of death certificate and its English translation, both to be authenticated by the Ministry of Foreign Affairs of the country where the death certificate was issued.
  • For a Filipina applicant divorced from a foreigner and if the data entry on name is damaged, a certified true copy of the petition for divorce and the divorce decree and its English translation, both to be authenticated by the Ministry of Foreign Affairs of the country where divorce was obtained or, if marriage is registered in the Philippines, one original copy of the marriage certificate from NSO and authenticated by the Philippines Department of Foreign Affairs (DFA) with annotation that the marriage has been declared null and void.
  • For a Filipina applicant whose marriage to another Filipino is annulled and if data entry on name is damaged,one originalcopy of the annotated Marriage Contract from NSO and authenticated by the Philippine Department of Foreign Affairs (DFA).
